Historical safety and loss trends are discussed to set the context for the topic. The major process hazards are reviewed togethe with the accident process. The legislative framework utilised by industry worldwide is presented with differences between regions being identified.
Hazard management systems and the supporting safety analysis techniques are presented. These include: Inherent Safety; Hazard Identification (HAZID); Layers of Protection Analysis (LOPA); Hazard and Operability Studies (HAZOP); Event Trees; Failure Mode Effect Analysis (FMEA). The concept of ALARP is presented and reinforced by example.
The development of management measures to prevent major accidents (Major Accident Prevention Policies, MAPP; Safety Management Systems, SMS) and the role of the legislator are presented.
Candidates will analyse a number of major accidents and use the taught material to identify the barriers which failed and thereby establish the underlying causes of the accidents.
