Case - On examination

  • As patient approaches you hear abnormal gait and on inspection foot drop is present.
  • Spine examination some muscle tenderness but no bony tenderness. Reduced range of spinal movement in particular lateral flexion.
  • Restricted SLR on left with positive nerve tension signs.
  • Increased tone on Left.
  • Weakness of extension of left big toe power 3 (Active movement against gravity but not resistance) L5 myotome.
  • Sensory loss/numbness on dorsum of left foot - light touch and pin prick (L5 dermatome).
