Miotics
Examples
Pilocarpine
Indications
- Constriction of the pupil (miosis)
- Management of raised intra-ocular pressure
Contraindications
Contraindications
- Acute inflammation of the anterior segment of the eye (see Mydriatics)
Mechanism
Pilocarpine reduces intra-ocular pressure by increasing the drainage of aqueous fluid. It does this by causing the ciliary muscle to contract. Contraction of the ciliary muscle provides traction on the trabecular meshwork pulling open the channels and helping it drain more freely.
Administration
Topical Eye Drops
Darkly pigmented irises may require higher doses of drugs to achieve result.
Adverse Reactions
- Blurred vision
- Local burning or stinging
- Headache (caused by ciliary spasm)
- Urinary retention from systemic absorption (rare)
