Mechanism

Nitric oxide (NO) is synthesised from L-arginine by nitric oxide synthase (NOS) in endothelial cells. The NO diffuses into smooth muscle cells where it binds and activates Guanylate cyclase, which synthesises cyclic GMP (cGMP) from Guanosine trisphosphate (GTP).

Sildenafil works by inhibiting phosphodiesterase 5 (PDE5). This prevents inactivation of cGMP. The increased concentration of cGMP increases smooth muscle relaxation, vasodilation occurs and results in penile erection as blood flows to the erectile tissue following activation of nitrergic nerves activated by sexual stimulation.
