Presenting Complaint:
- Site: Are there any areas that are spared? Has she noticed anything inside her mouth or around her genitals?
- Onset: Did the rash appear before the fever, or was she feeling generally unwell before the rash appeared?
- Character: Does it itch? Hurt? Bleed or weep?
- Radiation: Did the rash start in one location and spread, or did it all appear at once?
- Associated symptoms: Sore throat? Myalgia? Painful eyes? GI upset?
- Timing: Does the rash come and go, or is it there all the time? Is it getting worse?
- Exacerbating/Relieving Factors?
Don't forget that you can apply SOCRATES to most types of history!
Past Medical Hx: Has she had a rash like this before? Is she immunosuppressed? Has she had all of her childhood vaccines?
Medications/Allergies: Has she recently started or stopped any medications? Does she have known allergies? - Very important!
Family Hx: Has she been around anyone unwell or with a similar rash?
Social Hx: Travel and Sexual history should not be forgotten!
