What observations are important to record before and during a blood transfusion?
While different observations may be indicated depending on the situation, the mandatory recorded observations < 60 minutes before administering a blood transfusion are:
The same observations should also be made on completion of each unit of transfusion and 15 mins after the start of a new unit.
Observations may need to be recorded at more frequent intervals in unstable patients or if the patient seems to be having an adverse reaction.
Remember: In addition to observations, it is important to cross-check all patient details, the expiry date and details of the transfused component as well as consent for the transfusion.
