Pulmonary Embolism:
This ECG is of a pulmonary embolism i.e. a clot blocking the pulmonary arteries.
The defining feature if it is the SI, QIII, TIII pattern of abnormality.
There is a deep S wave in lead I, a deep Q wave in lead III and inverted T waves in lead III
Also sometimes on X-ray a wedge shape can seen distal to the thrombus
