Primary or Secondary Adrenal Insufficiency?
Following an abnormal short synacthen (low cortisol), you need to work out if it is primary or secondary adrenal insufficiency.
You do this using baseline serum ACTH:
- If it is high = primary insufficency (e.g. Addison's)
- If it is low = secondary adrenal insufficiency
