What does the X-ray show?

• Complex biconvex right sided pleural effusion with pleural enhancement
• Gas locules within

What is the diagnosis and differentials?

• Exudate/ transudate
• Empyema
• Parapneumonic effusion or effusion of another cause

What is the management?

• ABCDE, thoracic USS with diagnostic aspiration + chest drain
• CT Thorax
• Prolonged course of antibiotics
