Try to use a standardised format and consider the following:
- Pt details
- PA/AP
- Inspiratory effort
- Rotation and Penetration
- ABCDE (airway, breath/lung fields, cardiac, diaphragm and everything else (ie fracture/ ICD/PPM/NG tube/subcutaneous emphysema))
