Systematic review of personalised care planning for adults with chronic or long term conditions

This Cochrane review is funded by the Policy Research Programme in the Department of Health, England, and led by Professor Angela Coulter, University of Oxford. It aims to assess the effects of personalised care planning, as compared to forms of care in which the active involvement of patients in decisions about the treatment and management of long term conditions (at least in goal setting and action planning) is not explicitly attempted or achieved. It is considering effects in the domains of physical, psychological and psychosocial health, and capabilities for self-management, as primary outcomes.
